Weighing the Risks + Benefits of Weight Loss Shots
Discover the risks and benefits of weight loss shots like Semaglutide and Tirzepatide. Learn how they work and who qualifies for these injections.

Weight loss shots have changed the conversation about obesity faster than almost any medication in recent memory. They work — the trial results are genuinely remarkable, and for many people they have accomplished what years of effort could not. They also carry real side effects, a long-term commitment most people don't anticipate, and one consequence that rarely makes the headlines: a meaningful share of the weight lost isn't fat.
This guide covers what GLP-1 medications actually do, who qualifies, what the research says about risks, and why what you lose matters as much as how much.
Semaglutide and Tirzepatide Medications
Most of what people mean by "weight loss shots" comes down to two molecules.
Semaglutide is a GLP-1 receptor agonist sold under three brand names: Ozempic (type 2 diabetes, plus cardiovascular and kidney risk reduction), Wegovy (chronic weight management, plus cardiovascular risk reduction), and Rybelsus (an oral tablet for type 2 diabetes). This answers a common question — GLP-1 vs Ozempic isn't a comparison of two things. Ozempic is one brand of one GLP-1 drug. In December 2025, the FDA approved an oral 25 mg version of Wegovy, the first GLP-1 pill approved for weight loss. In its OASIS 4 trial — a relatively small study of 307 participants — the oral version produced a mean weight reduction of 13.6% at 64 weeks versus 2.2% on placebo.
Tirzepatide (Mounjaro for diabetes, Zepbound for weight management) is a dual agonist — it activates both the GLP-1 receptor and the GIP receptor. Zepbound is also the first medication approved to treat moderate-to-severe obstructive sleep apnea in adults with obesity.
A fuller GLP-1 list of GLP-1 drugs approved in the U.S. includes semaglutide, tirzepatide, dulaglutide (Trulicity), and liraglutide (Saxenda for weight management, and generic liraglutide for diabetes). Several older entries — exenatide (Byetta, Bydureon) and lixisenatide (Adlyxin) among them — have been discontinued in the U.S. market. And not all GLP-1 drugs are approved for weight management; some are diabetes medications where weight loss is a secondary effect.
Which GLP-1 Is Best for Weight Loss?
The only head-to-head trial gives a clear answer on the average. SURMOUNT-5 randomized 751 adults with obesity to tirzepatide or semaglutide for 72 weeks. Tirzepatide produced a mean weight reduction of 20.2% versus 13.7% for semaglutide, and a larger waist circumference reduction (18.4 cm vs 13.0 cm).
Averages aren't prescriptions, though. Tolerability, insurance coverage, dosing schedule, other conditions you're managing, and how your body responds all matter, and plenty of people do better on the drug that performed second in the trial.
What About GLP-1 Patches, Drops, and "Six-in-One" Supplements?
Short answer: GLP-1 patches are not real GLP-1 medications, and neither are the drops and "GLP-1 six-in-one" supplements marketed alongside them.
There is no FDA-approved transdermal GLP-1. The chemistry is the reason — peptides like semaglutide are large, water-loving, electrically charged molecules, which is exactly the profile that cannot cross the outer layer of skin. Products sold as GLP-1 patches typically contain herbal ingredients and stimulants such as green tea extract, garcinia cambogia, or bitter orange, which can cause jitteriness, elevated heart rate, and sleep problems without delivering anything resembling the drug on the label.
The same skepticism applies to research peptides. Products labeled "for research purposes only" or "not for human consumption" are sold outside the FDA approval process entirely, with no verification of what's in the vial or how much. The FDA has documented adverse events — some requiring hospitalization — tied to dosing errors with compounded GLP-1 products, and has warned that newer compounds like retatrutide and cagrilintide cannot legally be compounded at all. If cost is what's pushing you toward these options, that's a conversation worth having with a prescriber, not a reason to gamble on an unverified vial.
One quick clarification while we're here, since it's a common search: GLP-1 and GLP-2 are two different hormones produced from the same precursor protein. GLP-2 is involved in intestinal growth and repair, not appetite regulation, and GLP-2 drugs treat conditions like short bowel syndrome. It has nothing to do with weight loss.
How Do Weight Loss Injections Work?
What does GLP-1 stand for? Glucagon-like peptide-1 — a hormone your small intestine releases after you eat. So what is GLP-1 doing in your body already? It's a satiety and blood sugar signal, and it's remarkably short-lived: the natural hormone is broken down within about two minutes. GLP-1 medications are engineered versions that resist that breakdown, so a single weekly injection sustains the signal for days.
How does GLP-1 work once it's circulating? Four mechanisms, running at once:
- It prompts insulin release from the pancreas when blood sugar rises. Does GLP-1 lower blood sugar? Yes — that's what these drugs were developed for, and why they remain frontline diabetes medications.
- It suppresses glucagon, the hormone that pushes stored glucose into your bloodstream.
- It slows gastric emptying, so food stays in your stomach longer and you feel full longer.
- It acts on appetite centers in the brain, reducing hunger and, for many people, quieting the persistent food-related mental chatter they'd assumed was just willpower.
Tirzepatide adds GIP receptor activation to that list — a second incretin hormone pathway, which appears to be part of why its results run higher.
The benefits extend past the scale. In the SELECT trial, more than 17,500 adults with overweight or obesity and existing cardiovascular disease but no diabetes took semaglutide for a mean of about 40 months. The result was a 20% reduction in heart attack, stroke, and cardiovascular death — a benefit that doesn't appear to be explained by weight loss alone.
Who Qualifies for Weight Loss Injections?
FDA-approved criteria for chronic weight management are straightforward:
- BMI of 30 or higher (obesity), or
- BMI of 27 to 29.9 (overweight) with at least one weight-related health condition — type 2 diabetes, high blood pressure, high cholesterol, cardiovascular disease, and obstructive sleep apnea are among the most common.
These medications are not appropriate for everyone who meets those numbers. They are contraindicated for anyone with a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2, and they aren't recommended during pregnancy or while trying to conceive. A prescriber will also weigh a history of pancreatitis, gallbladder disease, severe gastrointestinal conditions, or diabetic retinopathy.
BMI is the gate, but it's a crude one — it can't tell muscle from fat, and it says nothing about where your body stores what it has. Body composition and waist measurement give a far more accurate picture of metabolic risk, and they're worth having in hand before the conversation starts.
Long-Term Use
This is the part most people underestimate.
Obesity is a chronic condition, and GLP-1 medications treat it the way blood pressure medication treats hypertension — by managing it, not curing it. Stop the drug and the underlying biology resumes.
The trial data is unambiguous. In the STEP 1 extension, participants who had lost a mean of 17.3% of body weight over 68 weeks regained 11.6 percentage points of it in the year after stopping — roughly two-thirds of the loss. Most of the cardiometabolic improvements drifted back toward baseline alongside it. In SURMOUNT-4, among participants switched from tirzepatide to placebo, 82% regained more than a quarter of the weight they'd lost within a year, while those who stayed on treatment held their results.
Real-world outcomes look somewhat gentler. A Cleveland Clinic analysis of patients who stopped semaglutide or tirzepatide found more stable weight trajectories than the trials predicted — partly because people who stop outside a study rarely stop everything. They switch medications, pause and resume, or shift to structured lifestyle approaches. The obesity treatment continues; only the drug changes.
Planning for years rather than months is the realistic posture. So is budgeting for it.
Reducing Use After Reaching Your Health Goals
If you and your clinician decide to step down or stop, a few principles improve the odds.
Taper rather than quit abruptly. Moving to a lower maintenance dose, or extending the interval between doses, is a common approach and gives you a chance to see where your weight settles before discontinuing entirely.
Build the habits while the medication is helping. The appetite suppression is a window, not the destination. Protein intake, resistance training, sleep, and consistent meal structure are what carry results forward, and they're far easier to establish while hunger is dialed down than after it returns.
Protect your muscle first. The more lean mass you kept during the loss phase, the higher your resting energy expenditure, and the less steeply the odds are stacked against you afterward.
Watch the data, not the mirror. Weight regain doesn't announce itself. Monthly body composition readings surface a reversal — especially fat mass creeping up while muscle mass slips — long before your clothes tell you anything.
Expect that returning is not failure. Many people cycle on and off these medications across years. Stopping and needing to restart is a normal feature of treating a chronic condition, not a personal collapse.
Risks of Using Weight Loss Injections
Are GLP-1s safe? For appropriately screened patients under medical supervision, the approved medications have a well-characterized safety profile — which is different from saying they're risk-free.
Common side effects are mostly gastrointestinal: nausea, vomiting, diarrhea, constipation, and reduced appetite. They cluster during dose escalation and usually ease with time. In STEP 1, 4.5% of participants discontinued semaglutide because of GI effects.
Serious risks appear in the prescribing information and warrant monitoring:
- Thyroid C-cell tumors — a boxed warning based on rodent studies; whether this translates to humans is unknown.
- Acute pancreatitis — a medical emergency, and one worth putting in context. It occurs in roughly 2 to 3 per 1,000 patients on these drugs, but the pooled trial data shows a nearly identical rate on placebo, so the attributable risk appears small. It remains on the label, and sudden severe abdominal pain warrants immediate care.
- Severe gastrointestinal reactions, including delayed stomach emptying severe enough that these drugs aren't recommended for people with significant gastroparesis.
- Gallbladder disease, which rapid weight loss independently raises the risk of.
- Acute kidney injury, usually secondary to dehydration from severe vomiting or diarrhea.
- Hypoglycemia, primarily when combined with insulin or sulfonylureas.
- Diabetic retinopathy complications in patients with existing eye disease.
- Serious allergic reactions, including anaphylaxis and angioedema — rare, but a reason to stop the drug.
- Pulmonary aspiration during anesthesia — tell any surgeon or anesthesiologist you're on a GLP-1.
- Reduced effectiveness of oral hormonal contraceptives with tirzepatide, particularly around dose increases. If this applies to you, ask about backup contraception.
Worth noting what came off the label: in January 2026, the FDA requested removal of the suicidal behavior and ideation warning from these medications after a meta-analysis of 91 trials and a cohort of more than two million users found no increased risk.
The Risk Nobody Mentions: Muscle and Bone
Here's the one that doesn't make the commercials. All weight loss costs you some lean tissue, and GLP-1 medications are no exception. The numbers vary widely across studies: some report lean mass accounting for 40–60% of total weight lost, others 15% or less. In STEP 1 and SUSTAIN 8 it ran roughly 39–40%; in the SURMOUNT-1 body composition analysis, closer to 25%.
Two pieces of context matter. First, that range is broadly in line with diet-induced weight loss and bariatric surgery — this isn't a GLP-1-specific defect, and participants' lean mass as a proportion of total body mass generally improved. Second, "lean mass" isn't the same thing as muscle: it includes organs, bone, and body water, so a drop in lean mass overstates how much actual muscle was lost.
Even with those caveats, the concern is legitimate. Skeletal muscle drives your resting energy expenditure, so losing it makes weight easier to regain and harder to lose the next time. For adults over 50, it stacks on top of age-related muscle loss already underway.
Bone is the adjacent question. Research presented at the 2026 AAOS annual meeting followed more than 73,000 matched patients who had both type 2 diabetes and obesity, and found osteoporosis diagnosed in 4.1% of GLP-1 users versus 3.2% of non-users over five years. The lead investigators were explicit that a retrospective analysis can't establish cause, the absolute difference is under one percentage point, and other work suggests GLP-1s may be neutral or even favorable for bone density. What's better established is simpler: rapid weight loss of any kind reduces bone mineral density.
The practical takeaway isn't to avoid these medications. It's that weight is the wrong thing to be tracking on them. A scale reading of 15 pounds down tells you nothing about whether those pounds were mostly fat or substantially muscle — and those two outcomes lead to very different places twelve months later.
How to Get Started
Start with a clinician, not a website. An obesity medicine specialist, endocrinologist, or primary care provider can assess whether you qualify, screen for contraindications, and manage dose escalation. Telehealth can be legitimate — but verify that the prescription is for an FDA-approved product from a state-licensed pharmacy, not a compounded or "research" version.
Get a baseline before your first dose. Body weight, waist circumference, and a full body composition reading — body fat percentage, muscle mass, visceral fat rating, bone mass. Without a baseline, you can't tell later whether your results are going well or quietly going sideways.
Build the muscle protection plan up front. Adequate protein spread across meals and resistance training at least twice a week are the two interventions with real evidence behind them for preserving lean tissue during weight loss. Start both in week one, not after a body composition reading alarms you.
Measure monthly, under the same conditions. Same time of day, similar hydration, same monitor.
